Recognizing the challenges faced by a growing number of people, Congress approved the National Family Caregiver Support Program as part of the 2000 Older Americans Act Reauthorization. This program calls for all states,working in partnership with Area Agencies on Aging and local community services for family caregivers,including:
- INFORMATION ABOUT RESOURCES
- ASSISTANCE TO FAMILIES IN LOCATING SERVICES
- CAREGIVER TRAINING, PEER SUPPORT AND COUNSELING
- RESPITE CARE PROVIDED
- SUPPLEMENTAL SERVICE ON A LIMITED BASIS
Case Management is a part of the community-based care system offered by the Area Agency on Aging. A trained staff person assesses the need and resources of the client and link him or her to a full range of short-term services. These services include:
- Homemaker
- Emergency Response
- Personal Assistance
- Limited Home Modification
- Meals and Health Mainenance
